Aging Life Care™ Manager

Requires a four-year degree and/or certification in Social Work, Nursing or related human services field.

Hiring for Sacramento Region (corporate office in Placerville)

Credentialed Care Manager – We currently have a part-time position for a care manager. Must have experience in care management/care coordination with a license or advanced degree preferred in gerontology, social work or nursing. Assigned caseload to be managed with a high degree of autonomy. Must have knowledge of working with clients and families in a psychosocial setting. Must be certified or have the ability to become certified within one of the three disciplines: NASW, CCM or CMC as a Certified Care Manager and must fulfill ongoing continuing education requirements indicated by the chosen discipline.
