Full Service Professional Care Management


PGCMs are credentialed professionals with degrees in Social Work, Gerontology, Nursing or Human Services. Our Certified Care Managers (CMC) have earned a national certification recognized after meeting the education, experience requirements and passing the national exam. PGCMs have extensive knowledge of costs, quality, government entitlements, eligibility, community resources and availability of services for older adults.

Elder Options' PGCMs :

  • Are members of the National Association of Professional Geriatric Care Managers (NAPGCM)
  • Conduct thorough assessments to identify issues, eligibility and needs.
  • Develop a plan of care that considers safety, cost & client wishes.
  • Monitor all community services including in-home provider employees.
  • Identify and assist with health, financial or legal issues.
  • Conduct family meetings to address family/client issues & lend support to family caregivers.
  • As necessary, help with transition to next level of care including all aspects of moving.
  • Are working on or have met the requirements necessary for the National Academy of Certified Care Managers (NACCM).

Elder Options, Inc. employs only the best in-home care providers. Prior to being hired, we require:

  • Complete background investigation
  • Application with education, employment, certifications and personal reference
  • DMV report
  • Auto insurance
  • Negative TB test
  • Face to face interview
  • Orientation & mentor training

CPR training is offered to all employees. Regular paid in-service training is given.

The Geriatric Care Manager (GCM) conducts the initial home visit and assessment with the client/family with ongoing supervision and monitoring for all clients. Home Care includes care for individuals with:

Alzheimer’s disease, dementia, Parkinson’s disease, COPD, cancer, ALS, heart disease, stroke and post-stroke, diabetes and lung disease among others. We work with clients as they begin to need assistance up to and including end of life care. Home care differs with each person but can include:

Assisting with bathing and personal care including lifting/transferring as needed.

  • Grocery shopping.
  • Meal planning, preparation and clean up.
  • Light housework.
  • Laundry, changing sheets/towels.
  • Medication reminders and reorders.
  • Safety and supervision when memory is an issue.
  • Providing transportation to errands, medical appointments, family outings.
  • Socialization i.e., going to the movies, shopping, playing cards, listening to music.
  • Feeding the animals, bringing in wood, watering the plants, baking cookies, making jam.
  • Respite care to relieve family caregivers on a regularly scheduled basis or as needed.

Day Shifts - 3 hour minimum per occurrence

Night Shifts - 12 hour night shift

  • Employee must be able to sleep 5-6 hrs./shift

24-hour Live In Shifts

  • Employee must be able to sleep 6-7 hrs./shift
  • Includes room & board

Holidays worked are paid and billed at time and half.

  • Establish, monitor & maintain monthly client petty
  • Cash fund account

Elder Options is a fee for service program. Fees vary based on services requested and are clearly defined.
Third party billing and reimbursement may be possible.
Third party billing (i.e. long term care insurance) is offered as a client service.

Medi-Cal Assisted Living Waiver (ALW) - Care Coordination

The Assisted Living Waiver program is operated by the California Department of Health Services (DHS) and covers nursing home type care services delivered to residents of residential care facilities for the elderly (RCFEs) and publicly funded housing sites.

Elder Options is approved as a Medi-Cal provider for the Care Coordination component of the ALW program in Sacramento county. People living in other counties can receive services if they are otherwise qualified, willing to relocate to one of the participating counties, and work with a care coordination agency such as Elder Options.

Participants must be eligible for full-scope Medical benefits and require a nursing facility level of care. Elder Options uses a standardized assessment tool to make that determination.

California Community Transitions - A New Program for Medi-Cal beneficiaries living in Skilled Nursing Facilities

Elder Options is now partnering with the Department of Health Care Services as a Lead Organization in a new demonstration project called California Community Transitions (CCT). This exciting program offers some new choices for people living in a Skilled Nursing Facility (SNF) who are on Medi-Cal, and want to explore the possibilities of moving back to the community.

The CCT program is a “Money Follows the Person” rebalancing demonstration funded by the Centers for Medicare and Medicaid Services. The program is designed to save government money by assisting people to return to their homes and communities with assistance from the state. Money that would have been spent keeping the person in a hospital can be spent assisting that person to stay at home with available services, usually for a lot less.

The CCT program is designed to assist those people who have lived continuously in a SNF for at least 6 months, who have been on Medi-cal for at least 30 days, and who want to transition back into their community, but don’t have to resources to do so.

The CCT program identifies eligible participants and assists them in transitioning out of nursing care and back into a community setting; either at home with family, in an apartment, or living with 2-3 others in similar situations. The program identifies supports and services available in the community, arranges training for family caregivers, makes modifications to homes and vehicles, and just about anything else someone would need to make the transition. A professional care manager works closely with the participant and their family, arranges needed services, then assists the participant throughout the process and for a year after the transition is made. The program is voluntary, saves money and helps people return to the community. Elder Options is pleased to be able to offer this service to our community.
