Utilize Experts - Who are Professional Geriatric Care Managers (PGCMs)?


PGCMs are credentialed professionals with degrees in Social Work, Gerontology, Nursing or Human Services. Our Certified Care Managers (CMC) have earned a national certification recognized after meeting the education, experience requirements and passing the national exam. PGCMs have extensive knowledge of costs, quality, government entitlements, eligibility, community resources and availability of services for older adults.
Elder Options' PGCMs :

  • Conduct thorough assessments to identify problems, eligibility and need.
  • Develop a plan of care that considers safety, cost & client wishes.
  • Monitor all community services including in-home provider employees.
  • Identify and assist with health, financial or legal issues.
  • Conduct family meetings to address family/client issues & lend support to family caregiver.
  • When necessary, help with transition to next level of care including all aspects of moving.
  • Have met the requirements necessary& passed the National CMC testing exam.

Professional Care Management Services Available

 

Office consultation, family meetings

Complete Assessment/Care Plan:

  • Initial home visit with individual/family*
  • Complete psychosocial assessment including*
    • Mini-mental memory exam
    • Falls assessment
    • Safety assessment of home
    • Depression scale
    • Psychological evaluation
    • Health issues & medications
    • Activities of Daily Living & IADLs
  • Development of goals, objectives & a written care plan.

Assistance with paying bills/money management

Accompany client to physician's appointments.

Monitoring home visits.

Written reports to client, family, physician, attorney

Medication/Monitoring

  • Reorder, physician calls, pick up meds.

*Included at no charge when home care employee(s) are utilized on a regular basis

Home Care Services Available

Day Shifts - 3 hour minimum per occurrence

Night Shifts - 12 hour night shift

  • Employee must be able to sleep 5-6 hrs./shift

24-hour Live In Shifts

  • Employee must be able to sleep 6-7 hrs./shift
  • Includes room & board

Holidays worked are paid and billed at time and half.

Petty Cash Fund

  • Establish, monitor & maintain monthly client petty
  • Cash fund account

Elder Options is a fee for service program. Fees vary based on services requested and are clearly defined.
Third party billing and reimbursement may be possible.

 

Assisted Living Waiver Project (ALWP) - Care Planning

The waiver project is operated by the California Department of Health Services (DHS) and covers nursing home type care services delivered to residents of residential care facilities for the elderly (RCFEs) and publicly funded housing sites.

Elder Options' Care Management department is approved as a Medi-Cal provider for the Care Planning component of the California Department of Health Services three year pilot project in Los Angeles, San Joaquin and Sacramento counties. People living in other counties can receive services if they are otherwise qualified, willing to relocate to one of the participating counties, and work with a care coordination agency such as Elder Options.

Participants must be eligible for full-scope or share-of-cost Medi-Cal benefits and require a nursing facility level of care. Elder Options uses a standardized assessment tool to make that determination.

New Mom

Call Elder Options to arrange help for a new mom as she and her baby come home from the hospital.

Our in-home care providers can welcome Mom home and help with:

  • laundry services
  • light housework
  • meal preparation
  • changing linens
  • taking care of other children in the family

 

Special three-hour, five-hour and eight-hour packages can be purchased for one day, one week or more starting at $75.00 a package. Gift certificates are available. Call us at (800) 336-1709 to arrange services.

24 hour dispatch center

...24 hour Emergency Response Center. Center personnel establish two-way contact with you, then follow a predetermined response plan.

Your local paramedics and/or family members or neighbors receive all vital information and arrive at your door within minutes.

If you keep your door locked, you can purchase a lock box for $45.00. Emergency Response Center workers will give the lock box code to local emergency response personnel, so they don't have to break down your door if you are incapacitated.

 

This vital protection is very affordable - a one time $60 installation fee and $35.00 a month thereafter.
