In-Home Provider Application
Elder Options, Inc. works with families to “live life fully every day”. This care managed home care agency assists client/families in remaining at home with help. Our credentialed care managers work with care providers to meet the needs and expectations of the client/family. Our in-home providers are integral part of the Care Team. You will assist with non medical duties such as client supervision, meal preparation, companionship, transportation, light housework, medication monitoring, ambulation, transfers, personal care and respite for family. We provide care throughout the Sacramento Region with most of our care providers working in the Folsom/El Dorado County/South Lake Tahoe area.

Criminal Background Check is conducted for every new Elder Options’ employee. Permission for this background check must be given in writing by each applicant prior to hire. (The form will be given to you to read & sign.)
TB Test. All applicants must submit a negative TB test prior to beginning work. We will accept a TB test taken within the last year. You may get a current TB test from:

DMV Report & proof of Automobile Insurance. Contact your auto insurance carrier or DMV to request one. This report is required for all applicants, no exceptions during hiring process.
Current driver’s license. Proof will be required during hiring process.
Please complete this application thoroughly & submit. During the hiring process you will need to provide:
- Valid Drivers’ License or Photo I.D. Card
- Social Security Card
- Recent DMV Report
- Proof of Auto Insurance
- Proof of a Negative TB test. (may be submitted at interview time)
- Proof of Current CPR certification (may submit within 30 days of employment)
- Copies of professional certifications, classes, licensure
- Background Check Authorization form signed.
