Our Care Partnerships & Programs
State of California (DHCS) – Medi-Cal Assisted Living Waiver Program (ALWP)
As an approved Care Coordination Agency (CCA) for Sacramento County, Elder Options has been a trusted partner in the Medi-Cal Assisted Living Waiver Program (ALWP) since its inception in 2006.
Our credentialed Care Managers and Registered Nurses (RNs) provide:
- Initial assessments and eligibility verification.
- Assistance with assisted living placement in approved ALW communities.
- Ongoing care coordination and monthly in-person monitoring.
This state-funded program offers an alternative to skilled nursing home placement, helping eligible Medi-Cal recipients transition to assisted living with financial assistance.
Respite Care Through Alta California Regional Center
Elder Options is a vendor-approved provider for the Alta California Regional Center, offering respite care for families caring for neurodiverse children and adults in El Dorado County, South Lake Tahoe, and Sacramento County.
Our Care Managers conduct home visits and develop personalized care plans to ensure high-quality, tailored respite care. Services include:
- Socialization and one-on-one interaction.
- Recreational activities and daily life skill support.
- Supervision and personal care assistance.
- Meal planning and preparation.
Respite care provides temporary relief for family caregivers, reducing stress and ensuring their loved ones receive quality care in a safe and supportive environment.
El Dorado County Family Caregiver Support Program (FCSP) & Multipurpose Senior Services Program (MSSP)
Elder Options partners with El Dorado County’s Family Caregiver Support Program (FCSP) and Multipurpose Senior Services Program (MSSP) to provide in-home care at no cost for eligible seniors and their caregivers.
Our services include:
- Light housekeeping and laundry.
- Meal preparation and grocery shopping.
- Personal care assistance (bathing, dressing, toileting).
- Companionship and supervision.
Through these programs, we help older adults remain safely in their homes while easing the burden on family caregivers.
Long-Term Care Insurance Assistance
For clients with long-term care insurance, Elder Options provides:
- Coverage review and claims assistance.
- Monthly case notes and billing support.
- Care Manager oversight to ensure benefits are maximized.

VA Home Health Aide & Respite Services
We are contracted with the US Department of Veterans Affairs (VA) to provide Home Health Aide and Respite services to eligible veterans. Referred by VA-authorized providers, these services are available at no cost to qualified individuals based on a VA needs assessment. Services include:
- Personal care assistance (bathing, dressing, hygiene).
- Meal preparation and light housekeeping.
- Supervision and companionship for safety and well-being.
- Short-term respite care for family caregivers.
Vital Link Emergency Response System for Veterans
Elder Options has partnered with Vital Link to offer discounted emergency response systems to veterans. Features include:
- 24/7 medical alert monitoring.
- No installation fee for eligible veterans.
- Simple push-button access to emergency assistance.
This service enhances independence and safety for older adults living at home.

Elder Options works closely with hospitals, home health agencies, hospice providers, and community organizations to provide a seamless continuum of care. Our trusted referral network includes:
